The survey discusses experiences of child abuse. Please read the form’s instructions before choosing to participate. You can leave optional questions unanswered or stop at any time.
The Google Form asks for your name, email and other personal information, and may require Google sign-in. It is not an anonymous survey. Review its consent and sharing questions carefully.
